The 2007 course is divided into three portions.

- Your name (Last, First)
- email address (you should be using the CUP email)
- Certification area
- Indicate that you have Federal fingerprinting* and both PA Background checks or clearance papers
- A phone number where I can reach now and between during the course
*Note new fingerprint testing clearance required. Pennsylvania Department of Education’s Background Clearance Page

Yes, I have both current clearances and I am applying for the federal printing. Must have affidavit stating that you have applied for the federal fingerprint notarized (notarizing can be done in the student union - 1st floor)
